As an older adult, being independent is likely a source of pride for you. Part of that independence is living at home, in your community, amongst family, friends and familiar surroundings on your own terms.
But sometimes… you need assistance with your health care, transportation, meals, or even medications.

Not familiar with PACE?
Read on to learn more about “PACE” a program to help individuals age at home. This is now available at LIFE COORDINATED Commonwealth PACE for eligible members.
What is PACE?
The Program of All-Inclusive Care for the Elderly, often just known as “PACE” allows older adults who would otherwise require nursing home care to live in their homes as long as possible by providing comprehensive services and care such as:
Our services are usually at no cost for seniors eligible for Medicaid and or both Medicare and Medicaid.
If you have Medicare only, you may pay a fee.
There's no deductible for any care, drug or service approved by your Care Team within the LIFE COORDINATED network.
Other that Emergency Services, it’s important to understand that members must receive ALL needed healthcare services including, but not limited to primary care and specialist care services from LIFE COORDINATED or a provider or organization authorized by LIFE COORDINATED. Members may be fully and personally liable for costs of unauthorized or out-of-PACE program services
Is PACE right for me?
How do I find out if I am eligible for PACE? First, check these requirements:
Do you live in one of the following counties in Western Kentucky – Calloway, Graves, Fulton, Hickman, Marshall and McCracken?
Are you 55+?
Do you meet the nursing home level of care as certified by the Commonwealth of KY?
Can you live safely in the community with help at the time of enrollment?
